The duplicate title form is one of the most important DMV forms. The DMV guide states that the form is used for a number of purposes. They can be used for a duplicate title (sections to be filled, 1 to 3), paperless title certification (sections to be filled, 1 to 3), transfer of title with duplicate title (sections to be filled, seller 1 to 5, buyer 6 to 10) and transfer of title with paperless title (sections to be filled, seller 1 to 5, buyer 6 to 10). Here is how to fill out the different sections of the Application for Duplicate Title form.
1. The License Plate/CF Number and the VIN/Vehicle ID Number
The section has to be filled out with the vehicle's license plate/CF number, as well as the vehicle identification number (VIN) or the ID number.
2. The Vehicle's Manufacturing Year and Make
This section requires the applicant to fill in the respective vehicle's year of manufacture as well as its make.
3. Registered Owner's Information
The section requires the true name of the registered owner or owners. It also requires the residence or the business address of the owner along with the apartment number, the name of the city and the state as well as the zip code. The driver license number also has to be filled in. If the mailing address is different from the residence or business address, this also has to be filled in.
4. Legal Owner's Information
This particular section is to be filled out with the name of the company or the individual who has a lien on the vehicle in question. Also to be filled in is the company or the individual's address with the apartment number, the city, state and zip code.
5. Missing Title Statement
This section has to be filled in with the applicant's signature, the date, the telephone number, the name of person signing for a company and the legal owner's name.
6. Registered Owner's Release of Ownership and/or Interest
In this section the applicant has to put in the owner's signature, the date and the daytime telephone number.
7. Legal Owner's Release of Ownership and/or Interest
This section requires the legal owner or the lien holder to put in a signature along with his or her name, the date, the county's name, the DMV officer's name and the officer's signature.
8. New Registered Owner's Information
The applicant has to put in the purchase price, date of purchase and equipment number. Also to be filled in are the new owner's full name, address and the driver license/ID card number along with the signature.
9. New Legal Owner's Information
The section has to be filled in with the true name of the new legal owner, the electronic lien holder ID number and his or her complete address.
10. Leased Vehicles
The section has to be filled with the lessee's address, only if it is different from the legal owner's address.
11. Vessel or Trailer Coach
In this section, the location of the trailer coach or the vessel has to be filled in.
12. Dealer's Release of Acquired Vehicle
The information to be filled in this section includes the name of the buyer, the name of the dealership along with the name of the agent and his or her signature, as well as the date of sale and the R/S number.
